Dental insurance plans are designed to share in the cost of your dental care, not to completely pay for those costs. Your dental coverage is not determined by your dental needs but by the agreement negotiated by your employer with the insurer. If you have a personal individual dental plan, your benefits are based on your specific plan. We are not responsible for how your insurance company handles a claim or for the amount that they pay or do not pay on a claim.
We will collect from you on the day of treatment the estimated amount that insurance is not estimated to pay. We will be happy to file the claim for you if you have provided all of your insurance information on the day of the appointment.
Regardless of your insurance company’s determination of usual and customary rates (UCR) or amount of assignment (allowable fees), you are required to pay the amount charged for your treatment. We will estimate your portion at the time of service. This is an estimate and any amount not paid by your insurance is payable by you.

Specific information about benefits available on your plan can be provided by the dental insurance company. Most companies print their customer service number directly on your insurance card. We encourage patients to know as much as possible about their dental plan prior to seeking treatment. Having that knowledge will prepare you to make informed decisions.
